Information We Collect

Grant Applicants — When you submit a grant application through our JotForm application portal, we collect personal information including but not limited to:

  • Child’s name, age, and diagnosis

  • Parent or guardian name and contact information

  • Medical and therapy history relevant to the grant request

  • Supporting documentation submitted as part of the application

This information is collected solely for the purpose of evaluating grant applications and administering our grant program.

Donors — When you make a donation through ApplePay, PayPal, Venmo, or Stripe, those platforms collect and process your payment information directly. Wolf Pups on Wheels does not store your payment information. We may retain your name and contact information for the purpose of donation records, tax documentation, and donor communication.

Website Visitors — Our website may collect standard non-identifying information such as browser type and pages visited through standard web analytics tools. We do not collect personal information from general website visitors unless you voluntarily submit it through a form or contact request.

How We Use Your Information

Wolf Pups on Wheels uses the information we collect for the following purposes:

  • To review, process, and administer grant applications

  • To communicate with applicants regarding the status of their application

  • To maintain accurate records of grants awarded

  • To communicate with donors regarding their contributions and tax documentation

  • To send organizational updates to those who have opted in to our email communications (you may unsubscribe at any time by using the unsubscribe link included in each email or by contacting us directly)

  • To fulfill our legal and financial reporting obligations as a registered nonprofit

How We Protect Your Information

We take the privacy of the families we serve seriously, particularly given the sensitive medical information shared in the grant application process.

  • Grant application data collected through JotForm is subject to JotForm’s privacy and security practices. We encourage you to review JotForm’s Privacy Policy at jotform.com/privacy.

  • Donor payment information is processed securely through ApplePay, PayPal, Venmo, and Stripe and is not stored by Wolf Pups on Wheels.

  • Access to grant application information is limited to authorized Wolf Pups on Wheels board members and staff directly involved in the grant review process.

  • We do not sell, trade, or otherwise share your personally identifiable information with third parties for marketing or commercial purposes.

Data Retention

Wolf Pups on Wheels retains personal information only for as long as necessary to fulfill the purposes for which it was collected. Grant application data, including sensitive medical information, is retained for a period of [three (3) years] following the final determination on the application, after which it is securely destroyed. Donor records are retained in accordance with applicable tax and financial reporting requirements. When personal information is no longer needed, we dispose of it using secure methods designed to prevent unauthorized access or disclosure.

Children’s Privacy

Wolf Pups on Wheels serves children with disabilities. In compliance with the Children’s Online Privacy Protection Act (COPPA), we do not knowingly collect personally identifiable information online from children under the age of 13 without verified parental consent. Information about a child submitted through our grant application process is provided by a parent or legal guardian and is used solely for grant evaluation purposes. We do not knowingly collect personally identifiable information directly from children.

Social Media and Public Content

When families consent to having their child’s story shared on our social media platforms or other public materials, we use only first names and share only the information explicitly approved by the parent or guardian. Consent is obtained separately from the grant application process. Families may withdraw consent at any time by contacting us directly.
